The TCP provides short-term specialised care and support to help you regain your independence and confidence sooner.

What is included?

Our Transition Care Program can include a range of services including:

Allied Health services

We offer dedicated allied health services such as occupational therapy and physiotherapy to help you regain your mobility and independence.

Nursing services

Our specialised clinical team can provide support including pain management, wound care and catheter support.

Home support services

Our experienced and compassionate team can deliver home support services including personal care, domestic assistance and meal preparation.

These services are specifically delivered to help you get back to living well and independently after your hospital stay. Our team will work with you to understand your individual needs, and provide you with a mix of services that will best help you achieve your goals.

How can I access the Transition Care Program?

The best thing to do is to talk to hospital staff while you’re still a patient in hospital. The hospital staff can arrange an ACAT Assessment, where an ACAT Assessor will visit you in hospital for an assessment. You can ask the hospital staff to arrange this assessment, or call My Aged Care on 1800 200 422.

It is important to organise this while you are still in the hospital or via My Aged Care before you discharge, as you are no longer eligible for this program once you’ve left hospital.

FAQs

How long is the program?

The average length of stay is usually about seven weeks, but if required, you can receive care for up to twelve weeks on the program. For the small number of care recipients who require more time in TCP, one extension of up to six additional weeks may be possible if approved by an ACAT Assessor.

Do I need a referral?

You will need to be referred after your assessment, it is important to organise this while you are in the hospital or via My Aged Care before you discharge.

An ACAT Assessor will visit you in hospital for an assessment. You can ask the hospital staff to arrange this assessment, or call My Aged Care on1800 200 422. 

Am I eligible?

You may be eligible for transition care if you are an older person and:

  • are a patient in a public or private hospital
  • have been told that you are ready to leave hospital
  • would benefit from receiving services for a short period of time.

You must be assessed by an Aged Care Assessment Team (ACAT) to establish your eligibility.

What about fees?

The Transition Care Program is predominantly funded with a completed ACAT Assessment and referral, with clients liable for a daily fee of $12.53 whilst on the program. All clients are informed of the fee associated with any given services at the time of assessment.
