Services
White Oak currently provides care services and clinical support services to a diverse range of clients in the community. These include, post acute care clients, the frail aged, clients with disabilities or special medical needs. White Oak will also
provide an extensive palliative care service.
Clients have direct access to management and clinical support 24 hours per day, 7 days per week via an on-call telephone service ensuring any issues can be dealt with immediately. Clinical staff will deal with all emergency care situations and direct staff in the appropriate treatment or referral to professional assistance to ensure that the best outcome is achieved for the client. The initial referral source is contacted as soon as possible to inform them of the health status of the client and variation in care or service delivery.
Upon discharge from the service, an audit is conducted inviting the client and family to comment on the quality of the service either verbally or in written form. Any causes of dissatisfaction are investigated, analysed and acted upon at Management Meetings and subsequent changes to policy and procedures implemented.
