When you are first admitted to White Oak Home Care Services for clinical or post acute care a Registered nurse will visit to assess your health care needs and develop a care plan with you. This will identify the support that you need and any equipment or additional specialist services you may require. Ongoing care will be coordinated by your case manager who will assist you when and if your care needs change.
Our team of Clinical Nurse Specialist, Registered Nurses and Enrolled Nurses are skilled in the areas of case management and clinical support and they will work closely with your GP and Specialists to ensure the best possible outcomes for you. No task is too big or too small for our staff and you can be assured that White Oak’s training program supports a team of clinicians who are experienced and highly skilled and updated in all aspects of community nursing.
White Oak‘s clinical team has a strong association with Perth hospitals both private and public, Aged Care Providers, GPs, DSC, Department of Veterans Affairs, and Respite Care Centres. We recognise the importance of easy access to services and how confusing it all can be when trying to find the most appropriate care for you, so we are very happy to help by sharing information and accessing other services to compliment your program of care.
Our palliative care team is available to support the client and family unit in the end stages of illness and to assist with symptom and pain management at home. We work closely with GPs and consultants respecting the client’s wishes to ensure a compassionate and holistic approach to support and counselling which encompasses the entire unit of and family/friends.
White Oak Is also approved by the Department of Veterans Affairs to deliver services to Veterans through the programs of Community Nursing Services and Veterans Home Care
