The company was established in 1998 and has grown extensively since then to become one of the most significant community care providers in Western Australia. White Oak is based in Joondalup and operates throughout the Perth Metropolitan area 24 hours per day 7 days per week supported by a team of clinicians with expertise in community care. Entitled clients receive the benefit of a range of services incorporating a holistic home-based care program.

The rehabilitation-centered approach to care delivered by clinicians and support staff, optimises the person's independence, confidence, and safety and improves his/her chance of remaining in the home environment. This problem, prevention/solving philosophy of care also helps to reduce unnecessary premature re-admission to acute health facilities.

White Oak utilises a case management model of care delivery, co-ordinating resources to meet the most complex client care needs. This model ensures the client receives a responsive, efficient, timely and individual package of care. The community medical team (Specialist, General Practitioner, Community Nurse and Allied Health) is an integral part of case management to clients in the community. White Oak has an excellent working relationship with many specialist and clinical practices in the metropolitan region.

White Oak provides a 24-hour on call service managed by Registered Clinicians. Admission to the service can occur within the initial 24-hour period following referral and also at weekends. For those clients with identified complex care needs a pre admission visit by a clinician can be arranged.
